In this pivotal role, you will work remotely as a behavioral health consultant within the Primary Care Mental Health Integration (PCMHI) framework. Your expertise will aid in providing high-quality, evidence-based interventions and consultations to both Veterans and Primary Care Providers (PCPs) as part of an interdisciplinary Patient Aligned Care Team (PACT). This position offers a competitive annual salary ranging from USD 60,000 to 95,000, commensurate with experience, plus a sign-on bonus.

Key Responsibilities\n

• Conduct intake assessments and identify problems to develop personalized treatment plans for patients.

• Utilize the latest version of the Diagnostic and Statistical Manual (DSM) to inform diagnoses and treatment goals.

• Provide individual, group, and family therapy, implementing therapeutic interventions that are culturally sensitive and tailored to the needs of diverse clients.

• Engage in continuous evaluation and follow-up to monitor patient progress and adjust treatment strategies as necessary.

• Collaborate effectively within an interdisciplinary team, contributing insights and fostering relationships that enhance patient outcomes.

• Maintain detailed and accurate clinical documentation using telehealth platforms and electronic health records.

Patient Population / Client Focus\n

The LMHC role at Great Valley Psychotherapy focuses primarily on adults and Veterans dealing with a variety of mental health challenges, including anxiety, depression, PTSD, and substance use disorders. Qualifications\n

• A Master’s Degree in Mental Health Counseling or a related field.

• Current licensure as a Licensed Mental Health Counselor (LMHC) in New York State.
