Key Responsibilities\n

• Conduct thorough assessments to understand each patient's unique psychosocial needs and collaboratively develop treatment plans that reflect these individual requirements.

• Facilitate short-term counseling sessions and crisis intervention when necessary, employing evidence-based practices to support patient resilience and recovery.

• Assist patients in navigating the complexities of entitlements and benefits by helping them complete forms and secure supplemental insurance that reduces reliance on emergency assistance programs.

• Actively engage with pharmaceutical companies and community resources to help patients obtain medication cost assistance, ensuring they receive necessary prescriptions without financial burden.

• Serve as an advocate for patients, enhancing their access to services provided by community agencies and fostering meaningful connections to support their mental health journey.

• Maintain detailed and accurate patient records in compliance with regulatory requirements while preparing necessary documentation and reports to support patient care.

• Coordinate transportation assistance for patients as needed and participate in case conferences to share insights and updates on patient progress during clinical meetings.

Patient Population / Client Focus\n

The ideal candidate will work with a diverse patient population facing various mental health challenges. This includes individuals struggling with anxiety, depression, trauma, and other emotional or psychological conditions. You will play a significant role in empowering patients to achieve their personal goals for mental well-being by providing empathetic and culturally sensitive care.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required to ensure a comprehensive understanding of mental health practices.

• Must hold an active, independent license as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• At least two (2) years of professional experience in mental health counseling, showcasing your ability to effectively address the needs of diverse populations.

Preferred Experience\n

While the minimum requirements include two years of experience, preference will be given to applicants with experience in telehealth or remote counseling environments, particularly those familiar with digital health platforms and technologies.

Compensation and Benefits\n
