Work Environment

This onsite position involves delivering care directly in the home care setting, allowing you to develop close relationships with your patients while providing them with the support they need in the comfort of their homes. The role requires hands-on interaction with patients, helping them through various therapeutic exercises, and implementing rehabilitation strategies tailored to their specific needs. Monday through Friday, from 9 AM to 5 PM, you will work in a supportive team atmosphere, collaborating with healthcare professionals and family members to prioritize patient recovery.

Key Responsibilities

• Conduct comprehensive assessments of patients to identify their physical therapy needs, establishing a clear understanding of their medical history and current conditions.
• Create individualized treatment plans incorporating evidence-based rehabilitation techniques aimed at maximizing recovery and improving overall patient function.
• Implement therapeutic exercises and modalities while educating patients about their health conditions and recovery processes, ensuring they understand their treatment plans and goals.
• Monitor patients' progress, making adjustments to treatment plans as necessary to enhance their recovery journey.
• Document patient interactions and treatment processes accurately in compliance with regulatory and organizational standards.
• Collaborate with interdisciplinary teams, including physicians and occupational therapists, to provide cohesive care and support for each patient.
• Provide important guidance to patients and families regarding long-term mobility goals and lifestyle adjustments.

Patient Population / Client Focus

In this role, you will primarily serve adult patients recovering from orthopedic surgeries, managing chronic pain, or undergoing rehabilitation for sports-related injuries. The diverse population in Astoria means that you will work with individuals from various backgrounds, each with unique needs and rehabilitation goals. Qualifications

• Must hold a Bachelor's degree in Physical Therapy from an accredited institution.
• Current and valid State License to practice as a Physical Therapist in New York.
• Minimum of 2 years of experience in outpatient physical therapy settings.
• Strong interpersonal skills and the ability to communicate effectively with patients, caregivers, and the healthcare team.

Preferred Experience

Compensation and Benefits
