This role offers a remote work setting, allowing flexibility while providing essential services to our patients from the comfort of their homes. While the core hours of operation are Monday to Friday from 9:00 AM to 5:00 PM, occasional evening shifts may be needed to accommodate patient demands.

Key Responsibilities\n

• Conduct thorough mental health assessments tailored to the specific challenges faced by elderly patients, incorporating personal, medical, and psychological histories.

• Develop, implement, and monitor individualized treatment plans that consider cognitive changes and the overall well-being of patients.

• Educate patients and their families about mental health disorders, treatment possibilities, and community resources, fostering empowerment and informed decision-making.

• Maintain accurate records of patient progress while upholding all legal and ethical documentation standards.

• Actively engage in team-based meetings and case consultations, contributing insights and sharing professional development experiences.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is mandatory.

• A current and valid license as an LCSW, LCPC, or LMFT in the state of Illinois is required.

• Demonstrated understanding of geriatric mental health disorders, treatment methodologies, and therapeutic approaches is crucial.

Compensation and Benefits\n
