Work Environment

This role is primarily onsite, allowing physical therapists to engage directly with patients in their homes. Key Responsibilities

• Conduct thorough examinations and evaluations of patients to determine functional limitations and disabilities.
• Develop, implement, and modify individualized treatment plans that align with best practices and evidence-based approaches.
• Engage in consultations with healthcare providers to ensure continuity of care across treatment phases.
• Document patient progress, treatment protocols, and any modifications to care plans in adherence to all regulatory requirements.
• Supervise and guide support staff, including Physical Therapy Aides, during patient treatments to maximize therapeutic effectiveness.
• Participate in community outreach and marketing initiatives to promote clinic services and encourage wellness within the community.
• Perform regular re-evaluations and provide patient education to enhance understanding of their treatment pathways.
• Maintain superior communication with colleagues, patients, and family members to foster an environment of transparency and trust.

Patient Population / Client Focus

The primary patient demographic we serve includes individuals with orthopedic conditions ranging from post-surgical recovery to chronic musculoskeletal problems. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
