Key Responsibilities\n

• Deliver comprehensive mental health support including education, counseling, and crisis intervention, tailored to patients' specific needs.

• Facilitate connections to community resources and support services for both patients and their families, enhancing their overall care experience.

• Conduct thorough assessments and develop personalized care plans that address mental health needs and promote recovery while preventing readmissions.

• Utilize electronic medical records (EMR) proficiently to document patient progress, update treatment strategies, and communicate seamlessly with interdisciplinary care teams.

• Educate patients and family members about transition plans and advocate for essential services, ensuring informed participation in care decisions.

• Maintain up-to-date knowledge of federal and state regulations, as well as private insurance policies, to facilitate appropriate care transitions.

• Demonstrate ethical decision-making and advocate for patient rights, including accessing protective services when necessary.

• Engage proactively in team meetings, providing insights and recommendations based on patient interactions to enhance care strategies.

• Perform other duties as assigned to support the overarching mission of the organization.

Patient Population / Client Focus\n

The mental health therapist will primarily support individuals grappling with mental health issues arising from acute, chronic, and terminal medical conditions. This role requires sensitivity to the unique psychological challenges faced by patients at varied stages of their health journeys, as well as a commitment to supporting their families through what can be an incredibly challenging time.

Qualifications\n

• A Master’s or Doctoral degree in clinical psychology, counseling, clinical social work, or a related field from an accredited institution (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

• An independent licensure in the state of Oregon or the state you reside in (e.g., LCSW, LMFT, LPC) is required.

• Proven ability to manage a caseload while effectively coordinating care with multiple providers and support staff.

• Proficiency in electronic medical records and effective email communication to facilitate patient interactions.
