Key Responsibilities\n

As a Licensed Professional Counselor at Mountain Springs, your day-to-day responsibilities will include:

• Collaborating with primary care providers to identify, treat, and manage mental health and psychosocial concerns, participating as an essential member of our integrated primary care team.

• Conducting patient intakes, focusing on comprehensive diagnostic and functional evaluations, and making informed recommendations for treatment goals and plans to streamline patient care.

• Delivering brief, focused therapeutic interventions to address immediate mental health needs effectively.
\n
• Advising primary care providers on patient eligibility for continued care within the clinic versus referrals to specialty mental health facilities, ensuring the best outcomes for patients.

• Monitoring patient progress, initiating follow-ups to assess treatment efficacy, and making necessary adjustments to intervention strategies as indicated.

• Developing and implementing personalized relapse prevention plans, fostering stability and coping skills among at-risk patients.

• Coordinating healthcare services related to behavioral health, facilitating connections with other treatment providers, both within and outside the clinic, with proper patient consent.

• Maintaining comprehensive documentation of patient progress and treatment plans in accordance with established standards and guidelines.

• Educating and empowering patients with self-management skills and lifestyle modifications to enhance their overall health and resilience.

• Providing consultation to clinic management and fellow team members about behavioral services and potential areas for evaluation and improvement.

Patient Population / Client Focus\n

Qualifications\n

To be considered for this position, candidates must possess the following qualifications:

• A Master’s Degree in Clinical Counseling or related field.

• A current, valid LPC (Licensed Professional Counselor) license in Texas.
