Orthopedic Physical Therapist - Full-Time Home Care

Compassion Elevated Therapy is actively searching for a skilled Orthopedic Physical Therapist to become an integral part of our dedicated team in Clifton, New Jersey. In this full-time, onsite position, you will utilize your expertise to provide exceptional physical therapy interventions that enhance patient outcomes and improve their overall quality of life.

Work Environment

This position is based in a home care setting, allowing you to work closely with patients in the comfort of their own homes. This setting provides a unique opportunity to create personalized treatment plans tailored to each patient's specific needs and lifestyle. You will collaborate with other healthcare professionals, including physicians and nurses, to ensure a consistent and holistic approach to patient care. The regular work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, providing a structured work-life balance.

Key Responsibilities

• Develop detailed and individualized treatment and care plans for each patient, ensuring that all interventions align with their rehabilitation goals.
• Collaborate closely with the patient's physicians and other healthcare providers to coordinate care and maintain a consistent level of treatment.
• Maintain accurate and timely documentation of patient progress and treatment strategies in our electronic medical record (EMR) system.
• Exercise independent judgment and utilize evidence-based practices to implement effective treatment combinations tailored to each patient's unique situation.
• Evaluate patients' progress and modify treatment plans as necessary based on their response and recovery trajectories.
• Comply with all applicable federal and state regulations governing physical therapy practice.

Patient Population / Client Focus

Your primary focus will be working with patients who require orthopedic rehabilitation, including those recovering from surgery, injury, or chronic conditions affecting their mobility and function. You will provide compassionate care tailored to the individual needs of each patient, helping them regain vital skills and improve their overall well-being.

Qualifications

• A Master's Degree or Doctorate in Physical Therapy from an accredited program is required.
• Active state licensure as a Physical Therapist in New Jersey.
• A minimum of two years of clinical experience in areas such as orthopedics, geriatrics, acute care, and outpatient settings.
• Proficiency in orthopedic assessment techniques and comprehensive patient care planning.
• Demonstrated strong communication, organizational, and problem-solving skills.

Preferred Experience

• Experience working with home care patients is highly desirable.
• Familiarity with documentation standards and regulations specific to the state of New Jersey and home health care.
• Training in specialized orthopedic treatment modalities and techniques is a plus.

Compensation and Benefits

We offer a competitive salary range of $110,000 to $130,000 per year, commensurate with experience, along with performance-based bonuses.
