Work Environment

As a Physical Therapist with Revive, you will work onsite, primarily in patients' homes, which allows you to deliver personalized care in a comfortable and familiar environment. Our home care model ensures that you will have a full caseload, providing you with a rewarding and fulfilling schedule while minimizing administrative responsibilities. Key Responsibilities

• Collaborate with the Director of Therapy Operations (DTO) to pinpoint clinical improvement opportunities that enhance patient care.
• Engage with employers, referral sources, and payers through consistent communication and networking to ensure streamlined patient access to services.
• Utilize clinical documentation systems for meticulous record-keeping, ensuring compliance with billing codes, charge capture, and adherence to care orders.
• Guarantee compliance with the Florida State Practice Act, Rules and Regulations, and Workers' Compensation regulations to uphold the highest standards of care.
• Drive metrics related to patient satisfaction and experience, emphasizing the importance of positive interactions and outcomes.
• Implement the Physical Therapy Guidebook and consistently deliver a “Perfect PT visit” to every patient.
• Manage clinical outcomes, collaborating with fellow therapists to address outlier cases effectively.
• Ensure efficient clinical workflows for optimizing patient intake, referral processing, and scheduling.
• Execute individualized therapy plans, focusing on early intervention and conduct same-day evaluations wherever possible.
• Promote a collaborative and professional environment that nurtures teamwork, high-quality service delivery, and patient safety.

Patient Population / Client Focus

You will work with a diverse range of clients, including those recovering from surgery, injuries, or debilitating conditions. Your expertise will be critical in developing tailored treatment plans that address each patient's unique challenges and goals. This inspection into their individual needs allows for a deeper connection, fostering trust and motivation in their recovery journey.

Qualifications

• Possession of an active Florida State license as a Physical Therapist.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Exceptional customer service skills, with the ability to approach patients and their families with professionalism, empathy, and tact.
• Strong problem-solving abilities that leverage logical thinking to identify key facts and recommend effective solutions.

Preferred Experience

• Experience working in home care or a community-based setting.
• Familiarity with electronic health records (EHR) systems and documentation standards.
• Proven track record of driving patient satisfaction and achieving positive clinical outcomes.

Compensation and Benefits

• Competitive salary ranging from USD 120,000 to 130,000 per year, commensurate with experience and qualifications.
