Work Environment

This position is based in the home care setting, allowing our Physical Therapists to work directly with patients in their residences. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical capabilities and functional limitations.
• Develop personalized treatment plans that address the specific needs and goals of each patient, incorporating evidence-based practices.
• Implement therapeutic interventions, track patient progress, and modify treatment plans as necessary based on outcomes and patient feedback.
• Document evaluations, treatment protocols, and patient progress accurately to comply with professional standards and regulatory requirements.
• Provide guidance and education to patients and their families about home exercises, safety modifications, and ongoing care.
• Collaborate with other healthcare providers to ensure continuity of care and a comprehensive approach to each patient's treatment.
• Participate in professional development through continuing education, training sessions, and in-house workshops.

As a Physical Therapist in home care, you will work primarily with a diverse range of patients including those recovering from surgery, managing chronic pain, or experiencing mobility limitations due to aging or medical conditions. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
