Position Overview

OnTrack Physical Therapy & Wellness is seeking a dedicated and passionate Physical Therapist to join our dynamic team in beautiful Fort Lauderdale, Florida. This is a full-time position focused primarily on onsite care within our home care facility. Work Environment

Located in vibrant Fort Lauderdale, our facility is designed to foster a collaborative team atmosphere. As a Physical Therapist, you will work closely with a multidisciplinary team of healthcare professionals who share a common goal of maximizing patient outcomes. Your typical work schedule will be Monday through Friday, from 8 AM to 5 PM, allowing for a healthy work-life balance in a scenic coastal city known for its therapeutic lifestyle.

Key Responsibilities

• Provide comprehensive evaluations for patients, utilizing a variety of standardized assessments and tools to ascertain their physical capabilities, limitations, and rehabilitation needs.
• Develop personalized treatment plans based on patient evaluations, collaboratively working with patients to set achievable goals that align with their recovery aspirations.
• Implement evidence-based treatment protocols tailored to individual patient presentations while continuously modifying approaches based on ongoing assessments and feedback.
• Document all patient care activities thoroughly and accurately, including initial evaluations, daily encounter notes, and discharge summaries, ensuring compliance with state and federal regulations.
• Act as a liaison between patients and other healthcare professionals, recognizing and documenting changes in patients' conditions and adjusting treatment plans accordingly.
• Engage in discharge planning and post-discharge follow-up to facilitate seamless transitions and ongoing care for patients.
• Maintain clear, respectful, and effective communication with patients and their families, as well as with medical staff and third-party payers.
• Participate in facility meetings, clinical training, and ongoing professional development to enhance clinical skills and contribute to a culture of continuous improvement.

Patient Population / Client Focus

You will primarily serve orthopedic patients, including those recovering from surgeries, sports injuries, chronic pain, and other musculoskeletal conditions. Your role is crucial in guiding these individuals through their rehabilitation journeys, fostering not only physical recovery but also emotional and psychological support as they navigate their treatment.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state licensure to practice as a Physical Therapist in Florida.
• Current BLS certification from the American Heart Association.

Preferred Experience

• Minimum of 2 years of experience in an outpatient orthopedic setting, demonstrating a strong foundation in therapeutic techniques and patient management.
• Experience with electronic medical records (EMR) systems for efficient documentation and patient care tracking.

Compensation and Benefits

OnTrack Physical Therapy & Wellness offers a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience and qualifications. Beyond base pay, our benefits package includes:
