As a full-time member of our onsite home care facility, you will play a crucial role in helping individuals regain their strength, mobility, and independence through personalized therapy. Work Environment

The work setting at Heal Motion Care is unique in that we focus on home care, allowing us to provide therapy in comfortable and familiar settings that cater to the specific needs of our clients. Our dedicated team collaborates closely with each other to ensure a seamless experience for patients receiving multiple services. You can expect a Monday to Friday schedule from 8 AM to 3 PM, which allows for work-life balance while delivering essential care to our clients.

Key Responsibilities

As a Physical Therapist at Heal Motion Care, your responsibilities will include:

• Conducting comprehensive evaluations for children and adults to assess their physical capabilities and limitations, setting a baseline for treatment.
• Developing and implementing individualized treatment plans that address the unique goals and challenges of each client.
• Regularly monitoring and reassessing clinical signs and symptoms to evaluate treatment efficacy, track progress toward goals, and modify treatment plans as needed.
• Documenting patient interactions meticulously, including SOAP notes and progress reports, using our electronic record system to ensure timely and accurate records.
• Providing education and support to patients and their families about the rehabilitation process, addressing any questions or concerns they may have.
• Collaborating with interdisciplinary teams to optimize care for clients needing multiple therapeutic disciplines.
• Upholding the values and culture of Heal Motion Care while acting as a representative of our mission in every interaction.

Patient Population / Client Focus

You will be working with a diverse patient demographic, ranging from children to adults, and individuals from various backgrounds. This role requires a strong desire to engage with patients across a spectrum of physical challenges, ensuring that our therapeutic approach is tailored and sensitive to each individual's needs.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's degree in Physical Therapy is required, with preference given to candidates holding a Master's or Doctorate of Physical Therapy (DPT) from an accredited program.
• Current and unrestricted license to practice Physical Therapy in the state of California.
• A willingness to conduct evaluations and provide treatments both in-person and via telehealth when clinically appropriate.
