Empower Health NP is seeking a dedicated and skilled Nurse Practitioner to join our dynamic team in Jamaica, New York. Work Environment

This position is based onsite, allowing you to engage directly with patients in their homes, thus providing exceptional, personalized care. Your workweek will average around 40 hours, with scheduled hours typically from 9:00 a.m. to 6:00 p.m. You may occasionally work evenings until 8:00 p.m. and take part in community outreach events on weekends. The role is a perfect blend of clinical practice and community involvement, ensuring that you remain at the forefront of patient care.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, focusing on both physical and mental health.
• Diagnose and manage a range of acute and chronic medical conditions, providing timely interventions and preventive care.
• Collaborate closely with community organizations to develop tailored health education programs, enhancing community health literacy.
• Deliver culturally sensitive care, advocating for patients' rights and ensuring equitable access to healthcare services.
• Coordinate referrals and follow-up care, utilizing a network of local healthcare providers to ensure continuity of care.
• Maintain thorough and accurate patient medical records, adhering to all regulatory standards and best practices.
• Participate actively in community outreach initiatives and health fairs to raise awareness and provide essential health screenings.
• Stay informed on community health trends and evidence-based practices to implement innovative care strategies.

Patient Population / Client Focus

Your role will encompass a wide range of patients, from children to the elderly, all from diverse backgrounds and cultural contexts. This position is particularly focused on underserved populations in Jamaica, New York, where access to quality healthcare has been historically challenging. By addressing the unique health concerns of these individuals, you will play a critical part in improving health equity.

Qualifications

• A Bachelor's Degree in Nursing is required; however, a Master's or Doctoral degree is preferred.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized entities, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current and unrestricted Nurse Practitioner (APRN) license in New York State is essential.
• Possession of both a DEA Number and NPI Number is required.

Preferred Experience

• A minimum of 3 years of clinical experience, preferably in a primary care or community health setting.
• Demonstrated ability to perform strong clinical assessments and diagnostic evaluations.
• Proficient in electronic health records (EHR) systems, enhancing documentation and patient engagement processes.
• Excellent interpersonal and communication skills, capable of building rapport with a diverse patient population.
• Ability to work independently while also being an integral part of a collaborative healthcare team.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience.
