Key Responsibilities\n

• Conduct thorough assessments and provide crisis intervention for patients experiencing emotional and behavioral challenges, tailoring treatment plans based on individual needs.

• Maintain clear and continuous communication with other healthcare professionals, including physicians, to report patient progress, address concerns, and collaboratively address treatment needs.

• Document clinical notes and treatment plans accurately and timely, adhering to department guidelines to ensure continuity of care.

• Collaborate with multidisciplinary teams to develop comprehensive treatment strategies, including discharge plans, based on psychosocial evaluations and established social work techniques.

• Foster relationships with local community resources and represent Trusted Hands Health Center at meetings and committees designed to address the social determinants affecting patient health.

Patient Population / Client Focus\n

The therapist will engage with a wide variety of patients, including those coping with depression, anxiety, trauma, and substance abuse issues. Our clients come from diverse backgrounds and age groups, and your empathetic approach will be pivotal in connecting with individuals seeking support in their mental health journeys.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution is required.

• Active, independent licensure in California as a Licensed Professional Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) is essential.

• A minimum of two (2) years of post-graduate professional experience providing mental health counseling services.

Preferred Experience\n

• Experience in working with telehealth platforms and delivering therapy services remotely is highly beneficial.
