Key Responsibilities

• Conduct comprehensive psychosocial assessments with patients to identify their emotional, social, and financial needs, collaborating closely with medical teams to facilitate holistic patient care.
• Provide both individual and group counseling sessions aimed at addressing common issues such as adjustment, grief, and coping mechanisms in relation to medical diagnoses and treatments.
• Work in partnership with community organizations to coordinate post-discharge services, thus ensuring continuity of care and smooth transitions for patients returning to their home environments.
• Engage families by offering emotional support and education regarding treatment options, available resources, and strategies to cope with the implications of healthcare challenges.
• Participate actively in hospital committees, providing expert insights into social work-related matters and contributing to policy development aimed at improving organizational practices.
• Offer crisis intervention services, demonstrating compassion and effective communication to support patients and families during critical moments.
• Develop, implement, and assess discharge plans, establishing connections for patients with appropriate community resources that will support ongoing care.

Qualifications

Compensation and Benefits
