Key Responsibilities\n

• Conduct comprehensive assessments and personalized treatment plans to meet the unique mental health needs of each patient.

• Maintain active communication with supervisors, direct care staff, and family members about patient progress to ensure a collaborative treatment approach.

• Organize and provide a tentative schedule for appointments, home visits, and follow-ups to monitor and evaluate patient progress effectively.

• Coordinate in-home observations by performing chart reviews and conducting staff interviews, as well as analyzing data to enhance patient care.

• Ensure that the program plan is implemented effectively and is tailored to each patient's requirements.

• Take part in Interdisciplinary Team Meetings to discuss patient follow-up and clinical reviews.
\n
• Document monthly and quarterly progress notes to track patient outcomes and inform treatment strategies.
\n
• Conduct training for staff and team members to ensure familiarity with the program plan requirements and documentation practices.
\n
• Provide ongoing support through follow-up interviews and observations to assist patients in achieving their therapeutic goals.

• Maintain accurate patient records to ensure compliance and accessibility for authorized parties.
\n
• Act as a liaison with the broader mental health community to advocate for patient needs and connect resources effectively.

• Collaborate in developing procedural training materials to enhance program efficacy.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Arizona.
