This full-time, onsite position involves providing exceptional care to patients within a home care setting. Work Environment

You will be working in a residential setting, visiting patients in their homes across Culpeper, VA, which allows you to provide tailored, one-on-one care. This environment requires not only clinical skills but also the ability to adapt treatments based on individual home conditions and patient capabilities. Key Responsibilities

• Conduct thorough patient evaluations including medical history and physical assessments to develop effective treatment programs tailored to individual needs.
• Create and implement personalized therapeutic exercises and rehabilitation programs, adjusting them as necessary based on patient progress and feedback.
• Provide skilled, therapeutic interventions in the patient's home environment, utilizing techniques designed to enhance mobility and relieve pain.
• Maintain comprehensive patient documentation through electronic health records, ensuring clear communication with referring physicians and other healthcare professionals.
• Guide and educate patients and their caregivers on the treatment process, progress expectations, and home exercise programs, fostering a collaborative approach to recovery.
• Engage in ongoing assessment and re-evaluation of patient progress to ensure optimal outcomes and make adjustments to treatment plans when necessary.
• Achieve key performance metrics, such as patient satisfaction scores and treatment efficacy, contributing to the overall success of our team and organization.

Patient Population / Client Focus

This position primarily focuses on orthopedic patients, but you may also treat individuals across various specialties including pediatrics, neurology, and sports medicine. You will interact directly with patients of different ages and functional abilities, necessitating a tailored approach to each individual's therapy plan. Your ability to create rapport and trust with patients will significantly influence their rehabilitation success.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid state license to practice as a Physical Therapist in Virginia.
• Minimum of two years of clinical experience in a physical therapy setting, ideally with a focus on orthopedic rehabilitation.
• Basic Life Support (BLS) certification from the American Heart Association.
• Exceptional communication skills—both verbal and written—to effectively convey treatment protocols and educate patients.
• Proficiency in word processing and electronic documentation systems to maintain patient records and reports.
• A passion for building deep, impactful relationships with patients and a commitment to helping them achieve personal health goals.

Preferred Experience

• Experience in home health care or community-based rehabilitation settings.

Compensation and Benefits

We offer a competitive salary range of $100,000 – $120,000 per year, complemented by a comprehensive benefits package.
