Key Responsibilities\n

• Conduct ongoing assessments to identify patient needs, establish treatment plans, and adjust strategies based on progress.

• Facilitate communication among supervisors, direct care staff, and family members to discuss patient progress and treatment updates.
\n
• Develop and share schedules for appointments, home visits, and related monitoring activities with supervisory staff.
\n
• Coordinate home visits for comprehensive patient evaluations, including observations and chart reviews, ensuring that treatment goals are met effectively.
\n
• Implement and monitor program plans to guarantee they are executed correctly and yield positive results.
\n
• Engage in Interdisciplinary Team Meetings and other relevant meetings to discuss patient treatments and collaborate on strategies.
\n
• Prepare and submit detailed monthly and quarterly progress notes reflecting patient advancements and areas for continued support.
\n
• Organize training sessions for staff on the implementation of program plans and documentation to ensure best practices are followed.
\n
• Provide follow-up support through interviews, home visits, and observations, ensuring ongoing assistance for each patient.
\n
• Maintain meticulous records to ensure timely access to information for concerned stakeholders, including families and treatment teams.
\n
• Act as a liaison between clients, the public, and the mental health community to support program development and community mental health initiatives.

• Assist in creating training materials and resources to enhance program efficacy for all individuals served.

Qualifications\n

Preferred Experience\n
