Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and groups, tailoring your interventions to meet diverse client needs.

• Perform initial patient assessments, evaluate treatment plans, and effectively triage care based on clinical priorities.

• Document interactions, treatment updates, and interdisciplinary care discussions clearly and ethically within our electronic health record system.

• Provide ongoing case management for assigned clients, coordinating care with other healthcare professionals and community resources to ensure holistic support.

• Utilize evidence-based treatment modalities, including Cognitive Behavioral Therapy (CBT), to facilitate the development and execution of individualized service plans.

• Engage in regular consultation and collaborative meetings, sharing insights and contributing to a cohesive treatment approach across disciplines.

• Maintain a patient-centered focus that addresses physical, psychological, and socio-emotional needs across all age groups.

Qualifications\n

• Must hold a valid license as a LMFT, LPC, or LCSW in the State of Connecticut.

Compensation and Benefits\n

This position offers a competitive salary range of USD 70,000 to 100,000 per year based on your experience and qualifications.
