Work Environment

The work setting for this role is onsite at local schools in Guilderland, NY, providing a unique opportunity to engage directly with students and educational staff. You will collaborate closely with teachers, school administrators, and other healthcare professionals to ensure that students receive appropriate health assessments, interventions, and follow-up care in a supportive educational atmosphere. Key Responsibilities

• Coordinate comprehensive patient care activities, prioritizing tasks such as patient education, counseling, and discharge planning, while effectively directing and delegating responsibilities to the healthcare team.
• Conduct phone triage and assessments for acute clinical issues as needed, providing timely medical advice and prescribing appropriate treatments per established protocols.
• Assist in developing, implementing, and evaluating care standards and protocols within the school environment, ensuring that all practices align with institutional and regulatory guidelines.
• Perform thorough medical, surgical, and psychosocial health assessments, including documenting relevant findings and maintaining accurate problem and medication lists for continuity of care.
• Order and interpret diagnostic tests (laboratory and imaging) to formulate differential diagnoses, utilizing both subjective and objective data collected during patient evaluations.
• Facilitate patient admissions, transfers, and discharges for students requiring additional medical attention, while coordinating necessary inpatient and outpatient follow-up care.
• Engage in routine health screenings, immunizations, and health promotion activities to proactively address public health concerns within the school community.

Patient Population / Client Focus

This position primarily focuses on a diverse group of students, providing healthcare services to children and adolescents from various backgrounds and developmental stages. You will address an array of health issues, from chronic conditions to behavioral health concerns, fostering an environment where students feel safe and supported in seeking care.

Qualifications

• NP Board Certification as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP).
• Possession of an active DEA License and current New York State Nurse Practitioner License, along with a valid New York State Registered Nurse (RN) License.
• Certifications in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS).
• Strong organizational skills and attention to detail, with the ability to efficiently manage multiple tasks and prioritize patient needs in a dynamic school environment.
• Proficient in computer systems and electronic medical records, with a willingness to adapt to new applications quickly.

Preferred Experience

• Experience working in a school-based health program or pediatric setting is highly desirable.
• Familiarity with health education and community outreach initiatives.
• Proven ability to collaborate effectively with multidisciplinary teams.

Compensation and Benefits

The Nurse Practitioner role offers a competitive salary range of USD 150,000 to 160,000 annually.
