Key Responsibilities\n

• Engage with assigned clients in both office and community settings, conducting assessments and providing therapeutic interventions tailored to those coping with significant mental health issues.

• Collaborate with interdisciplinary teams to develop and implement integrated care plans, ensuring that clients receive the comprehensive services they need.

• Conduct evidence-based screenings and assessments, creating detailed evaluations when required and incorporating the input of clients and families in care planning.

• Provide a variety of therapeutic services, including individual, group, and family therapy, focusing on empowering clients and facilitating their growth and recovery.

• Monitor client progress, coordinating with medical and other service providers to ensure continuity of care and appropriate responses to changing needs.

• Participate in case management duties, such as crisis intervention, stabilization support, and ongoing evaluations to ensure effective treatment outcomes.

• Maintain compliance with medical records standards and submit timely reports and data as required by organizational protocols.

• Engage in continuous professional development by participating in training, workshops, and quality improvement committees to enhance service delivery.

Patient Population / Client Focus\n

The primary focus of this role will be the hospice population, including clients experience symptoms of major mental illnesses and severe emotional disorders. You will work closely with individuals and their families navigate the complexities of their circumstances, providing not only therapeutic support but also a listening ear during challenging times.

Qualifications\n

• A Master's degree in Social Work (MSW) or a related field is required.

• Active LCSW licensure in the state of Connecticut is mandatory.

• Demonstrable expert knowledge in assessment, diagnosis, and treatment of mental health issues, including risk assessment and crisis management.

• Strong verbal and written communication skills, coupled with excellent organizational and planning abilities.

• Cultural competency and understanding of diverse client backgrounds are essential.

Preferred Experience\n

Compensation and Benefits\n
