Key Responsibilities\n

• Conduct comprehensive assessments of individual patients to identify their unique needs and challenges in a timely manner.

• Draft and share a tentative schedule of appointments, home visits, and any observations necessary to ensure effective treatment tracking.

• Coordinate and perform home visits as required for in-depth assessments and observations, documenting findings meticulously.

• Oversee the effective implementation and ongoing review of program plans, ensuring they align with patients' requirements and therapeutic needs.

• Participate actively in interdisciplinary team meetings, offering insights and suggestions based on clinical observations and assessments.

• Regularly complete detailed progress notes and reports on patient statuses on a monthly and quarterly basis.

• Design and organize training sessions to ensure that staff members are fully adept in implementing program directives and maintaining accurate documentation.

• Provide follow-up visits and interviews to verify continued support for patients, ensuring that their needs are met effectively.

• Maintain organized records that ensure timely access to information by relevant parties in line with legal and ethical standards.

• Act as a representative for Best Care Counseling within the public and mental health communities to promote patient advocacy and program development.

• Contribute to the formulation of training materials aimed at improving processes and overall program quality for client care.

Qualifications\n

• Possession of active, unrestricted licensure as a Professional Counselor in the state of Pennsylvania.
