Key Responsibilities\n

• Document same-day clinical notes for clients at risk of utilizing crisis services, ensuring timely and accurate records.

• Complete critical incident reporting forms within a 24-hour timeframe as necessary.
\n
• Participate in after-hours support on an alternating basis to assist with care coordination efforts.
\n
• Work directly with participants to proactively prevent crises through individualized support plans.
\n
• Maintain updated crisis alerts for seamless coordination with county crisis services.
\n
• Manage the transition of clients from hospital or inpatient services with coordinated care, assisting with discharge planning alongside hospital staff.
\n
• Utilize crisis de-escalation skills to support individuals effectively and provide milieu support as the designated Clinician of the Day.
\n
• Conduct thorough risk assessments and develop safety plans based on individual client needs.

Compensation and Benefits\n
