Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients, integrating their mental health histories with their current emotional and environmental factors.

• Engage in individual, family, and group therapy sessions designed to enhance coping skills and emotional resilience using evidence-based therapeutic methods.

• Implement therapeutic frameworks such as Cognitive Behavioral Therapy, Solution-Focused Therapy, and Trauma-Focused Therapy tailored to client needs.

• Develop and refine individualized treatment plans with measurable goals, regularly evaluating client progress and adjusting interventions as necessary.

• Work collaboratively with interdisciplinary team members, including psychiatrists and psychologists, to ensure a harmonious approach to client care.

• Provide crisis intervention services, including risk assessments and safety planning, to address urgent emotional needs effectively.

• Maintain thorough and timely documentation that meets ethical and legal standards, ensuring continuity of care and adherence to best practices.

• Facilitate psychoeducational workshops, support groups, and therapeutic activities tailored to enhance clients’ understanding of their mental health.

Patient Population / Client Focus\n

This role focuses primarily on individuals receiving hospice care, who often face mental health challenges related to chronic illnesses and end-of-life issues. You will be helping clients cope with grief, loss, and existential concerns, as well as providing support to families navigating these difficult transitions.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience working in mental health, particularly in outpatient or hospice settings.

Compensation and Benefits\n
