Work Environment\n

This position offers the flexibility of a remote work setting, allowing social workers to connect with clients and families from the comfort of their homes. While most interactions will be facilitated via telehealth platforms or phone calls, occasional in-person visits may be required to assess client conditions or engage with family members. The typical working hours are Monday to Friday, from 8:00 AM to 4:30 PM, with the potential for evening or weekend hours based on client needs and availability.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the needs and challenges of seniors, considering emotional, mental, and social factors that affect their well-being.

• Collaborate closely with clients and their families to develop individualized care plans aimed at achieving specific goals while respecting personal preferences.

• Provide counseling and emotional support to seniors and their families, guiding them through life transitions, illness, and end-of-life issues.

• Advocate for clients, ensuring their rights are protected and that they have access to necessary community resources.

• Perform regular follow-up visits, either in-person or remotely, to monitor client status, adjust care plans as needed, and ensure a safe living environment.

• Educate clients and their families about available community resources, support groups, and programs to foster social engagement and enhance overall quality of life.

• Document all client interactions, assessments, care plans, and progress notes accurately in the electronic health record (EHR) system.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n
