Key Responsibilities\n

• Monitor patient progress and initiate follow-up communication to assess ongoing needs and adjust treatment strategies as required.

• Develop personalized relapse prevention plans for clients, aiding them in maintaining stable functioning and improving their overall well-being.

• Assist primary care providers in identifying patients who may benefit from specialized mental health services, ensuring smooth referrals when appropriate.

• Document all patient interactions, progress, and treatment plans accurately and maintain compliance with the Code of Ethical Conduct for Clinical Professionals.

Qualifications\n

Compensation and Benefits\n

St. Charles Counseling Services offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
