Work Environment

As a Physical Therapist with us, you will work in the comforting and familiar setting of our patients' homes, providing individualized care tailored to their unique needs. This onsite role entails a full-time schedule from Monday to Friday, amounting to 40 hours per week. You will collaborate closely with other healthcare professionals and have the opportunity to maximize patient outcomes through a coordinated approach to rehabilitation. Key Responsibilities

In your role as a Home Care Physical Therapist, your responsibilities will include:

• Conducting thorough evaluations to assess patients' physical functions and limitations, establishing baselines to guide treatment planning.
• Developing comprehensive treatment plans in cooperation with healthcare providers, ensuring adherence to recommended guidelines and protocols.
• Delivering specialized therapeutic interventions tailored to a diverse patient population, including orthopedic, pediatric, neurologic, and sports medicine cases.
• Utilizing advanced therapeutic techniques and modalities to effectively decrease pain and enhance patients' functional abilities in alignment with their goals.
• Demonstrating strong communication skills by consulting with, educating, and training patients and their caregivers about home exercises and mobility strategies.
• Tracking patient progress diligently, adjusting treatment plans as necessary, and documenting findings accurately to support ongoing care.
• Fostering a supportive environment by maintaining excellent customer service, contributing to the overall effectiveness of the team to achieve meaningful patient outcomes.

Patient Population / Client Focus

Your work will involve collaborating with a diverse array of patients, each facing unique challenges in their recovery journey. You will leverage your clinical skills to address the needs of patients with challenging conditions such as post-operative recovery, chronic pain, pediatric developmental issues, and sports-related injuries. Building strong relationships with patients will be fundamental, encouraging them to actively participate in their recovery process and achieve their goals.

Qualifications

The successful candidate will meet the following qualifications:

• Graduation from an APTA or CAPTE-approved physical therapy program.
• A Bachelor's degree in Physical Therapy; possessing a Doctorate in Physical Therapy (DPT) is highly regarded.
• A current state license to practice as a Physical Therapist in Florida.
• Certification in Basic Life Support from the American Heart Association.
• Proficient written and verbal communication skills necessary for effective patient interaction and documentation.
• Basic computer skills for documentation and record-keeping.

Preferred Experience

We strongly prefer candidates who possess at least two years of experience in a clinical setting that emphasizes patient-centered care, particularly in home health or outpatient physical therapy environments. Familiarity with electronic health record (EHR) systems and knowledge of evidence-based therapeutic practices will also be beneficial.
