Work Environment

This position is primarily onsite, where you'll be visiting patients in their homes. The home care setting allows you to build one-on-one relationships with patients, tailoring rehabilitative programs to suit their living conditions and personal needs. Key Responsibilities

• Conduct comprehensive evaluations of patients, including physical assessments and the review of medical history, to develop individualized treatment plans that meet each patient's unique needs.
• Clearly communicate evaluation findings and treatment objectives to patients and their families, engaging them in active goal-setting for their rehabilitation journey.
• Implement tailored treatment protocols based on your assessments and stay updated with evidence-based practices, adapting techniques as required to fit the patients' evolving conditions.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure thorough patient management, recognizing when it is necessary to revise care plans or seek further medical consultation.
• Document patient progress and treatment activities accurately and comprehensively, ensuring follow-up care aligns with insurance requirements and clinical standards.
• Manage your daily schedule effectively to meet productivity expectations while maintaining professionalism in all patient interactions.
• Take an active role in department meetings, contributing to the development and achievement of our objectives and participating in community outreach programs to enhance our clinic's visibility and community relations.
• Engage in continuing education and self-directed learning to stay abreast of advancements in therapy techniques and regulations, thereby enhancing your skills and competency.
• Encourage a positive patient experience by being adaptable and responsive to their needs, ultimately helping to remove barriers to access to care.

Patient Population / Client Focus

As a Physical Therapist at Reliant, you will work with a diverse patient population, including elderly individuals recovering from surgery, children with developmental delays, and adults regaining mobility after injuries. Our focus is to empower patients to achieve their rehabilitation goals through personalized therapeutic interventions. You will be an integral part of their recovery journey, providing support not only physically but emotionally as well.

Qualifications

• Bachelor's degree in Physical Therapy, with a Doctorate in Physical Therapy (DPT) preferred.
• A valid Physical Therapy license in the state of Florida.
• Current BLS certification from the American Heart Association for Healthcare Providers.
• Physical therapists with at least 2 years of experience in a clinical setting are encouraged to apply.

Preferred Experience

While a minimum of 2 years of clinical experience is required, candidates with specialized experience in home health care will have a competitive advantage. Practical experience with patient evaluation methodologies and treatment plan implementation in a home care context is highly desirable.

Compensation and Benefits
