Key Responsibilities\n

• Serve as a key liaison between clients, their families, and outpatient service providers, ensuring seamless communication and comprehensive care coordination.

• Engage with insurance companies for pre-certification and utilization management, advocating effectively for our clients’ needs.
\n
• Participate actively in discharge and aftercare planning, contributing valuable insights and recommendations to support client transitions.
\n
• Provide therapeutic meal support to clients, modeling and discussing healthy relationships with food to foster well-being.
\n
• Deliver empathetic and affirming care tailored to each client's unique circumstances, considering their intersecting identities and personal narratives.

Patient Population / Client Focus\n

The ideal candidate will focus on delivering social work services to older adults and their families in a hospice setting. Qualifications\n

• Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
