Key Responsibilities\n

Qualifications\n

• A Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current licensure as an LCSW, LCPC, or LMFT in the state of Illinois.

• Experience and proficiency in working with individuals facing co-occurring medical conditions.

• Strong history of delivering integrated care and collaborating with healthcare professionals across disciplines.

• Outstanding interpersonal and communication abilities essential for building rapport with patients and team members.
