Key Responsibilities\n

• Conduct ongoing assessments to evaluate and address the unique needs of each patient.

• Facilitate open communication with supervisors, direct care staff, patients, and their families to ensure a comprehensive understanding of each individual's progress.

• Develop and propose a strategic schedule that outlines appointments, home visits, and necessary observations to monitor patient advancements.

• Coordinate in-home visits that fulfill specific criteria for observational purposes, including chart reviews and staff interviews.

• Implement and uphold the efficacy of programmed interventions according to established guidelines.

• Participate actively in Interdisciplinary Team Meetings, providing insights and updates as needed.

• Document comprehensive progress notes on a monthly and quarterly basis to track changes in patient status.

• Organize and facilitate training sessions to ensure team members are well-versed in program implementation and documentation procedures.

• Perform follow-up assessments, home visits, and check-ins to guarantee continued support tailored to individual patient needs.

• Maintain accurate and readily accessible records to ensure a streamlined information-sharing process.

• Serve as a liaison within the mental health community, working collaboratively to develop resources and support systems.

Qualifications\n

• Active and unrestricted licensure as a Professional Counselor (LPC) in Texas.

• A strong foundation in family therapy, couples therapy, trauma-informed care, or substance misuse counseling is highly desirable.
