Key Responsibilities\n

• Manage an active caseload of clients, building rapport and trust through consistent engagement.

• Conduct regular sessions to monitor client progress, adjust treatment plans as needed, and prepare for safe and thoughtful discharges.

• Design, implement, and review person-centered treatment plans tailored to the unique needs of each client.
\n
• Document client interactions and treatment outcomes diligently in compliance with regulatory standards.

• Perform thorough intake assessments to identify client needs and establish appropriate treatment paths.

• Participate in individual supervision and team meetings to ensure a cohesive approach to client care.

• Collaborate with other healthcare professionals to deliver comprehensive care coordination.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s Degree in a mental health related field.

• Active licensure as a Licensed Professional Clinical Counselor (LPCC).

Compensation and Benefits\n
