Key Responsibilities\n

• Conduct comprehensive mental health assessments to evaluate the influence of co-occurring medical conditions on patients' mental well-being.

• Facilitate therapeutic sessions, including individual and group therapy, employing evidence-based approaches tailored to each patient's unique situation.

• Provide crisis intervention services during mental health emergencies, ensuring patient safety and appropriate follow-up care.

• Educate patients on the interaction between mental health and physical health, empowering them with strategies for self-management of their conditions.

• Maintain complete and accurate patient documentation in accordance with all legal, ethical, and organizational standards.

Patient Population / Client Focus\n

You will be primarily focused on serving patients with complex, co-occurring conditions. These include individuals suffering from chronic pain syndromes, autoimmune diseases, and various substance use disorders. By providing integrated care, you will play a key role in helping this vulnerable population access the resources and support they need to achieve better mental and physical health outcomes.

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Valid and current licensure as a LCSW, LCPC, or LMFT in the state of Illinois.

• Proven ability to address the mental health needs of individuals with co-occurring medical conditions.

• Strong skills in developing and implementing integrated care strategies, with a history of collaborating effectively with medical professionals.

• Excellent verbal and written communication skills, along with strong interpersonal abilities.

Compensation and Benefits\n
