Key Responsibilities\n

  Conduct comprehensive assessments and evaluations to understand and address the mental health needs of your clients.

  Facilitate ongoing communication with supervisors, direct care staff, and other stakeholders to provide updates on patient progress and care plans.

  Develop and share tentative schedules with supervisors regarding client appointments, home visits, and necessary observations to track and support client progress.

  Organize home visits for thorough in-home assessments, ensuring all required observations, chart reviews, and data analyses are conducted effectively.

  Guarantee that the program plans for clients are implemented with fidelity and monitor outcomes regularly to adjust strategies as needed.

  Engage in Interdisciplinary Team Meetings to collaborate with colleagues on patient care strategies and share insights from your assessments.

  Document and complete monthly and quarterly progress notes to maintain comprehensive and accessible records for all clients.

  Conduct follow-up interviews and home visits to provide ongoing support for your clients, adapting care strategies as necessary.

  Serve as a liaison with the broader mental health community to foster relationships that support client development goals.

  Assist in crafting training materials and procedures to improve program efficacy and enhance staff competency in deliverables.

Patient Population / Client Focus\n

  A Master's or Doctoral degree in Counseling, Psychology, or a related field.
