Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans that encompass academic, professional, and personal growth dimensions for clients from diverse backgrounds.

• Provide medication management support by assisting clients with medication prompts and adherence reminders, and ensuring timely refills and proper storage.
\n
• Facilitate transportation for clients to attend medical appointments, as well as offer guidance on navigating public transportation systems to encourage independence.
\n
• Support the daily living skills of clients by assisting them in meal planning, shopping, and cooking, while employing educational strategies to enhance their competencies.
\n
• Monitor client moods and behaviors, documenting observations accurately and maintaining compliance with best practices and agency safety protocols.
\n
• Collaborate closely with interdisciplinary team members, ensuring seamless communication regarding client progress and needs.
\n
• Maintain a caseload that reflects a busy yet fulfilling workload, providing ongoing feedback and support to clients throughout their therapy journeys.

• Adhere to all legal, regulatory, and contractual obligations, ensuring that all services comply with program certifications and licensing requirements.

Qualifications\n

• Master’s degree in counseling, social work, clinical psychology, or a related field.

• Active LCSW, LPC, LCPC, LMHC, or LMFT license in the state of California is required. Note: We do not accept applicants with an associate license due to Medicare billing restrictions.

• Strong technological skills, particularly proficiency in Microsoft Office Suite, to facilitate documentation and communication within our systems.

• Excellent organizational skills and the ability to manage multiple cases in a fast-paced environment.

• Demonstrated self-direction and capability to work independently with minimal supervision.

Preferred Experience\n

• Experience working with geriatric populations is a plus but not required.

• Previous experience in community mental health settings is highly desirable.
