Work Environment

This position is based onsite at our rehabilitation facility, featuring state-of-the-art equipment and a collaborative team of healthcare professionals dedicated to patient rehabilitation. Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing their medical history and performing specific assessments to tailor individualized treatment plans.
• Effectively communicate evaluation findings and treatment options to patients and their families, encouraging their participation in goal-setting and rehabilitation processes.
• Demonstrate expertise in implementing treatment protocols aligned with the latest evidence-based practices, adapting as necessary to meet the unique needs of each patient.
• Consult with interdisciplinary healthcare teams to ensure integrated care and modify treatment plans based on patient progress and changes in condition.
• Document and maintain accurate records of patient care activities, ensuring clarity in progress notes and discharge summaries while meeting accreditation standards.
• Manage time effectively, meeting productivity benchmarks and maintaining an organized schedule for patient appointments.
• Engage in community outreach and marketing initiatives to promote access to care and increased awareness of our services.
• Actively participate in continuing education to enhance personal and team competency, with the opportunity to serve as a clinical instructor for aspiring PT students.
• Contribute to departmental goals by reviewing performance metrics and collaborating with team members to enhance patient access and satisfaction.
