Key Responsibilities

Your role will encompass several key responsibilities, including:

• Conducting comprehensive evaluations to assess patient histories and perform targeted assessments, followed by formulating personalized treatment plans.
• Effectively communicating evaluation results and treatment plans to patients and their families, involving them in goal-setting for better adherence and motivation.
• Implementing treatment protocols based on individual patient evaluations and adapting these protocols in response to patient progress and changing healthcare needs.
• Collaborating with a multidisciplinary team of healthcare professionals to optimize patient care and document any changes in patient conditions that require modifications in treatment strategies.
• Coordinating follow-up care and discharge planning, ensuring that patients have a clear understanding of their care pathways.
• Maintaining accurate documentation of patient encounters, including evaluations, progress notes, and discharge summaries to capture the continuum of care.
• Demonstrating proficient time management skills to meet productivity expectations while being punctual for meetings and other engagements.
• Engaging in departmental goals and strategically participating in marketing initiatives to enhance community awareness of our services.
• Actively participating in professional development and continuing education, serving as a mentor for students or less experienced staff when applicable.

Patient Population / Client Focus

This position focuses on a diverse patient demographic, predominantly dealing with orthopedic cases that require rehabilitation following surgical procedures, injuries, or chronic pain conditions. You will provide care that enhances mobility, reduces pain, and promotes recovery in a home environment, allowing your patients to receive support in a familiar setting.

Qualifications

To apply for this position, candidates must meet the following qualifications:

• A Bachelor's degree in Physical Therapy is required.
• Doctorate in Physical Therapy (DPT) is preferred.
• Valid license to practice physical therapy in New Jersey is mandatory.
• American Heart Association's BLS for Healthcare Providers certification.
• Strong interpersonal skills and the ability to communicate effectively with patients and families.

Preferred Experience

While prior work experience is not required, candidates who have experience in home health or outpatient orthopedic settings will be given preference. Experience working with diverse populations and understanding varying socio-economic factors impacting health care delivery is a plus.

Compensation and Benefits
