Key Responsibilities\n

• Communicate effectively with physicians and members of the healthcare team to relay patient progress, address concerns, and collaborate on care plans.

• Maintain meticulous and timely clinical records, including detailed clinical notes and progress assessments, following established department policies.

• Collaborate in the development of treatment plans, ensuring that psychosocial assessments guide interventions and discharge strategies tailored to client needs.

• Provide crisis interventions and emotional support, assessing and treating patients facing acute emotional and behavioral challenges.

• Engage and consult with family members and support systems, educating them about treatment options and resources available to enhance the client’s support network.

Patient Population / Client Focus\n

You will primarily work with individuals and families navigating the complexities of hospice care, encompassing a wide spectrum of emotional and psychological challenges. Your role will be crucial in facilitating effective communication among all stakeholders involved in the patient’s care, fostering an environment of understanding and support.

Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

• A minimum of 3 years of relevant experience in an outpatient setting, with a strong emphasis on providing therapeutic services.

• Proficient technical skills, particularly with electronic medical records and Microsoft Office Suite.

• Ability to work independently, showcasing self-direction, organizational skills, and time management.

Preferred Experience\n

While 3 years of experience is required, preference will be given to candidates who have significant expertise in hospice care settings or have worked with similar populations requiring specialized psychosocial support. Familiarity with multidisciplinary team collaboration will be advantageous.

Compensation and Benefits\n
