Key Responsibilities

• Conduct comprehensive evaluations of patients, which include assessing medical history, performing necessary assessments, and developing tailored treatment plans.
• Clearly communicate evaluation results and treatment recommendations to patients and their families, ensuring they are involved in the goal-setting process.
• Implement treatment protocols based on patient assessments, current health conditions, and best practices within the evolving healthcare environment.
• Collaborate with other healthcare professionals as necessary and recognize when changes in patient health require modifications to the care plan or additional consultations.
• Manage discharge planning and follow-up care, diligently documenting patient interactions with accuracy and efficiency.
• Meet departmental productivity standards by effectively managing time and maintaining an organized schedule, participating in community outreach initiatives to promote our services.
• Support the achievement of departmental goals by complying with regulatory standards and engaging in quality improvement initiatives.

Qualifications

Compensation and Benefits
