Work Environment\n

This position is primarily a remote role, allowing you the flexibility to conduct assessments and therapy sessions from your home office while still being an integral part of our team. You will engage with clients through telecommunication methods, ensuring accessibility to services. Although the role is remote, you will participate in regular virtual meetings and training sessions to ensure a consistent and collaborative approach to client care.

Key Responsibilities\n

• Conduct thorough assessments of clients' needs over the phone or through telehealth platforms, gathering relevant information for accurate case management.

• Provide timely crisis intervention services, offering emotional support and strategic coping strategies to clients and their families during high-stress situations.

• Facilitate referrals to appropriate community resources and services, ensuring clients receive comprehensive care tailored to their specific circumstances.

• Guide clients in engaging with beneficial activities and resources that promote mental and emotional wellness.
\n
• Maintain detailed, accurate records of client interactions and progress, preparing reports as necessary for case management and treatment planning.

• Monitor and identify signs of child abuse or neglect and perform risk assessments when needed, in accordance with state regulations and ethical guidelines.

• Actively participate in training, supervision, and organizational meetings to enhance your skills and contribute to the ongoing education of fellow team members.

Patient Population / Client Focus\n

Your primary focus will be serving patients in hospice care, frequently dealing with issues surrounding end-of-life decisions, bereavement, and emotional distress related to chronic illness. Clients may experience feelings of anxiety, depression, or overwhelming grief, and as an LCSW, you will be pivotal in addressing these concerns with empathy and professionalism.

Qualifications\n

• A Master’s Degree in Social Work (MSW) or a related field is required.

• You must possess a current Licensed Clinical Social Worker (LCSW) credential in Pennsylvania.

• You should have at least two years of experience in clinical social work, preferably within a healthcare or hospice setting.

• Demonstrated expertise in assessment, diagnosis, and treatment of psychological issues, including crisis intervention and risk assessment.

Compensation and Benefits\n
