Key Responsibilities\n

• Provide clinical services directly to clients within hospice and community settings, addressing the symptoms of severe emotional and mental health disorders.

• Engage meaningfully with clients and their families to establish trust and rapport, ensuring they feel supported throughout their care journey.

• Conduct thorough assessments using evidence-based methodologies to develop integrated care plans tailored to individual client needs.

• Facilitate individual, group, and family therapy sessions to promote recovery and resilience, thus enhancing the overall therapeutic experience.

• Participate actively as a member of interdisciplinary teams to coordinate care and improve patient outcomes, ensuring effective communication among team members.

• Monitor client progress and adapt treatment plans as necessary to keep pace with their evolving needs and to ensure their goals are being met.

• Engage in crisis intervention as required, utilizing established protocols to provide immediate support to clients in distress.

• Ensure compliance with industry regulations and professional standards while maintaining the confidentiality and dignity of all clients.

• Contribute to quality improvement initiatives, sharing insights and experiences that may enhance service delivery and team performance.

• Participate in on-call coverage as assigned, ensuring that clients receive timely care whenever needed.

Patient Population / Client Focus\n

Your primary focus will be on individuals who are navigating the challenges of serious mental health issues and emotional disorders within the context of hospice care. This includes clients facing end-of-life matters, requiring sensitivity, patience, and a deep understanding of the mental and emotional ramifications of their circumstances.

Qualifications\n

• A valid LCSW license is required.

• A Bachelor’s degree in Social Work is mandatory, with a Master’s degree being preferred for more advanced practice and understanding.

Compensation and Benefits\n
