Active Physical Therapy is seeking a dedicated and compassionate Full-Time Physical Therapist to join our team in Woodbridge, Virginia. Work Environment

This position is onsite, emphasizing direct patient care in the comfort of their homes. As a full-time member of our team, you will work Monday through Friday, allowing for consistent scheduling and predictable patient interactions. Key Responsibilities

• Conduct comprehensive evaluations that include reviewing patient histories, performing specific assessments, and documenting treatment recommendations clearly and effectively.

• Communicate evaluation findings and proposed treatment plans to both patients and their families, ensuring that they are actively involved in goal-setting and recovery processes.

• Implement treatment protocols that are tailored to each patient's unique presentation and needs, continuously adapting methods based on their progress and the latest healthcare practices.

• Collaborate with other healthcare professionals to optimize patient care, documenting any changes in the patient's condition and adjusting the treatment plan as necessary.

• Oversee discharge planning and follow-up care, ensuring accurate and timely documentation that reflects the patient's rehabilitation journey, including evaluations, progress notes, and discharge summaries.

• Exhibit excellent time management skills by maintaining an accurate schedule and meeting productivity targets, contributing to overall department goals.

• Engage in community outreach and marketing efforts, promoting the importance of patient access and engagement in their care.

• Facilitate the continuous improvement of departmental objectives while complying with healthcare regulations and accreditation standards.

• Pursue ongoing education and professional development opportunities, while also serving as a mentor to students and new staff members.

• Encourage a positive patient experience through flexible and adaptable approaches that meet diverse patient needs.

Patient Population / Client Focus

In this role, you will be primarily serving patients receiving home care, which includes a variety of demographics and health conditions. Your therapeutic approach will directly impact patient outcomes by enhancing mobility, reducing pain, and improving overall function in their own living spaces. You'll build meaningful relationships with patients and their families, emphasizing continuity of care within their familiar environment.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.

• Licensure to practice as required by the state of Virginia.

• Current certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.

• While prior work experience is not required, a passion for patient care and a commitment to teamwork are essential.

Preferred Experience

• Previous experience in a physical therapy setting, particularly in home care, is a plus but not mandatory as we offer mentorship and support to new graduates.

• Familiarity with electronic health record (EHR) systems and documentation best practices is advantageous.
