Work Environment

The role of Physical Therapist at Guided Grace is based in onsite home care environments, meaning you will travel to patient residences to provide therapy. This setting allows for individualized care tailored to the patient's home environment, promoting comfort and familiarity. You will engage in a Monday through Friday work schedule, with rotating weekend shifts, allowing for a stable yet dynamic work routine that accommodates patient needs while supporting work-life balance.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations, utilizing techniques that include range of motion assessments, strength testing, and functional mobility reviews.
• Develop comprehensive treatment plans that are aligned with physicians' orders, incorporating therapeutic modalities, exercises, and adaptive equipment tailored to each patient's specific goals and conditions.
• Maintain detailed and accurate patient records, documenting evaluations, treatments, and progress reports in a timely manner using professional terminology that justifies therapeutic necessity and associated charges.
• Manage patient care effectively by supervising Physical Therapy Assistants (PTAs) and support staff, ensuring all team members are contributing to the patient's overall treatment plan.
• Collaborate with an interdisciplinary team, including physicians, occupational therapists, and speech-language pathologists, to create a cohesive approach in delivering optimal patient care outcomes.
• Evaluate the effectiveness of therapeutic interventions, adjusting treatment protocols as necessary based on patient feedback and clinical outcomes.
• Engage in continuous education and professional development opportunities to stay current with industry best practices and innovations in physical therapy.
• Provide constructive feedback to the therapeutic team and management to enhance patient care quality and service efficiency.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
