Work Environment

This role is primarily onsite, operating in the home care environment where you will provide services in the comfort of our patients' homes. Our setting allows for a unique therapist-patient dynamic; you'll directly observe how patients interact within their home contexts, tailoring interventions to fit their daily lives. Working Monday through Friday, from 8 AM to 5 PM, you will benefit from a structured schedule that allows for work-life balance while delivering continuous care to those in need.

Key Responsibilities

• Conduct thorough evaluations for each patient, including a comprehensive review of medical history and performance assessments to establish individual treatment plans.

• Implement treatment protocols and modalities customized to each patient's condition, ensuring effective and timely adjustments in care based on their progress and response.

• Document all aspects of patient care meticulously, including initial evaluations, treatment plans, daily notes, and discharge summaries to maintain compliance with healthcare regulations and provide continuity of care.

• Facilitate communication and collaboration with physicians, other healthcare professionals, and family members to optimize patient outcomes and ensure coordinated care.

• Engage in the ongoing education and training required to maintain clinical excellence and regulatory compliance, ensuring the highest standards of patient safety and care.

• Assess the need for referrals to other medical or therapeutic services and guide patients and families on follow-up care strategies to extend their rehabilitation journey.

Patient Population / Client Focus

The primary focus will be on patients requiring physical rehabilitation due to orthopedic issues, including those recovering from surgeries, injuries, or chronic conditions impacting mobility. You will provide hands-on care and education that empowers patients to regain their independence and improve their functional capabilities. Your work will directly affect patients in Virginia and help them navigate their rehabilitation journeys, enhancing their quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy; Doctorate degree in Physical Therapy (DPT) is preferred.

• A current state license to practice physical therapy in Virginia.

• Certification in Basic Life Support (BLS) recognized by the American Heart Association is required.

• Demonstrated knowledge of current evidence-based practices in physical therapy.

Preferred Experience

• A minimum of 2 years of experience in an outpatient, home care, or orthopedic settings is preferred.

• Experience developing and implementing individualized therapy plans tailored to diverse patient needs and goals.

Compensation and Benefits
