Key Responsibilities\n

• Conduct ongoing assessments to evaluate the mental health needs of patients and formulate individualized treatment strategies.

• Communicate effectively with supervisors, direct care staff, caregivers, and family members to review client progress and make necessary adjustments in treatment plans.
\n
• Provide supervisory updates on scheduled appointments, home visits, and other observations to ensure a comprehensive approach to client care.
\n
• Organize and conduct home visits to better understand clients' environments, enabling tailored interventions.
\n
• Facilitate and ensure the proper implementation of program plans, evaluating their efficacy over time.
\n
• Actively participate in Interdisciplinary Team Meetings to share insights and collaborate on best practices for patient care.
\n
• Document monthly and quarterly progress notes to track client development and outcomes.
\n
• Lead training sessions for staff on the implementation of treatment plans and necessary documentation requirements.
\n
• Ensure continued client support through follow-up interviews and home visits to address ongoing needs.
\n
• Maintain organized records to ensure information is accessible for all relevant stakeholders, reinforcing accountability and transparency.

• Serve as a liaison with the public and the mental health community to support shared goals for development and awareness.

• Assist in developing training materials aimed at enhancing program efficiency and client satisfaction.

Patient Population / Client Focus\n

As an LCPC at Aid Sphere Solutions, you will work with a wide-ranging client base, including adults and youth grappling with various mental health challenges. Your focus will be on creating safe spaces for healing, facilitating therapy for issues such as trauma, substance abuse, and family dynamics. Your expertise will guide clients through their journeys toward healthier lives.

Qualifications\n

• Possession of a Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of two years of clinical counseling experience is required, with a preference for backgrounds in family therapy, trauma-focused therapy, or substance misuse counseling.

• Strong understanding of legal and ethical standards governing mental health practice.

• Proficiency in using Electronic Medical Records (EMR) software and related healthcare technologies.

Compensation and Benefits\n
