Work Environment

The role is primarily onsite, as a visiting physical therapist providing care in the homes of our patients. Key Responsibilities

• Evaluate, plan, and implement individualized treatment programs for patients referred by physicians, aligning with prescribed care to achieve optimal functional outcomes.
• Administer a range of physical therapy techniques and modalities in patients' homes, such as exercise therapy, manual therapy, and education, tailored to their specific conditions.
• Demonstrate exemplary customer service, ensuring that patients feel supported and empowered throughout their treatment process.
• Collaborate with a multidisciplinary team to maximize patient outcomes through evidence-based care, sharing insights and strategies to enhance overall effectiveness.
• Consult, educate, and train patients and their caregivers on care techniques, progression plans, and home exercise regimens to promote sustainable recovery.
• Meet minimum utilization goals and other business objectives while actively participating in clinic-wide initiatives to enhance patient care and service delivery.

Patient Population / Client Focus

As an Orthopedic Physical Therapist, you will engage with a diverse population, handling cases that include orthopedic injuries, pediatric conditions, neurological disorders, and sports-related issues. Our home care model allows us to offer tailored interventions in a comfortable and familiar environment, ultimately improving adherence to therapeutic activities and enhancing patient outcomes.

Qualifications

• A valid Doctorate degree in Physical Therapy (DPT) and a current state license in Physical Therapy are required.
• At least two years of hands-on experience in a physical therapy setting, with demonstrated proficiency in evaluating and treating a variety of musculoskeletal and neurological conditions.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong written and verbal communication skills to effectively interact with patients, caregivers, and team members.
• Proficiency in using electronic health records and other technology to document patient progress and coordinate care.
• A deep commitment to building rapport and trust with patients to promote adherence to treatment plans and functional goals.

Preferred Experience

• Experience in home health physical therapy or familiarity with home care protocols is highly desirable.
• Background in sports medicine or pediatrics will be an added advantage, given our diverse patient population.

Compensation and Benefits
