Key Responsibilities

• Conduct Comprehensive Assessments: Undertake timely psychosocial assessments for patients and their families, applying your understanding of human behavior and psychosocial factors that could impact treatment outcomes.
• Clinical Intervention: Provide supportive counseling, connecting patients and families with community resources, facilitating family meetings, and aiding with complex decisions such as hospice placements and advance care directives.
• Case Management: Efficiently handle and prioritize a caseload that includes high-risk patients, ensuring effective management while coordinating closely with case managers to promote successful discharge planning.
• Adhere to Legal and Ethical Standards: Ensure compliance with relevant laws and ethical practices during all patient interactions and interventions, including addressing concerns around patient rights and suspected abuse.
• Documentation: Maintain up-to-date and precise documentation as mandated by hospital policies, including progress notes and interdisciplinary care plans, to ensure continuity of care.
• Education and In-Service Training: Provide training sessions to hospital staff on psychosocial aspects of patient care, contributing toward organizational knowledge and patient support initiatives.
• Quality Improvement Participation: Engage in ongoing assessments and development of quality indicators to enhance the delivery of patient care and services.
• Operational Support: Engage in extracurricular activities such as Social Work Month events, support groups for cancer patients, and educational programs for staff and family members.

Patient Population / Client Focus

You will primarily focus on supporting individuals and families facing medical challenges, including those with chronic illnesses, mental health issues, and life-limiting conditions. Your expertise will guide patients through emotional and practical challenges associated with their health journeys, aiming to provide them with tools and resources to cope effectively.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Minimum of two years of experience in a medical setting, with at least one year in an oncology or cancer-related environment.
• Effective communication skills in English, both written and verbal, to engage with patients, families, and healthcare professionals.
• Demonstrated ability to perform independently while also collaborating effectively within a multidisciplinary team.
• Strong problem-solving, planning, and analytical skills to navigate complex patient situations.
• An unwavering commitment to providing excellent customer service and ensuring superior patient experiences.

Preferred Experience

• Experience using electronic health records (EHR) systems for documentation and patient management.
• Familiarity with community resources and networks for referrals and patient support.

Compensation and Benefits

The annual salary for this position ranges from USD 120,000 to 130,000 , reflecting the qualifications and experience of the candidate.
