Position Overview

Gotham Enterprises Ltd is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our dynamic outpatient practice at the Center for Advanced Cardiac Care (CACC) located in Brooklyn, New York. In this full-time onsite role, you will operate under the leadership of the Chief Nurse Practitioner and work closely with the CACC Medical Director within our renowned Division of Cardiology. You will play an essential role in providing high-quality care to our diverse patient population, making significant contributions towards improved patient outcomes in cardiovascular health.

Key Responsibilities

As a Nurse Practitioner in our facility, you will engage in a variety of tasks that are integral to providing holistic and effective patient care:

• Conduct thorough patient histories and perform both comprehensive and targeted physical assessments to establish a clear clinical picture.
• Order and interpret essential diagnostic tests, utilizing your clinical judgment to diagnose and manage both chronic and acute cardiac conditions.
• Develop and implement goal-directed treatment plans that focus on health maintenance, patient education, and the application of evidence-based medical and nursing interventions.
• Collaborate closely with attending physicians and nursing staff to adapt treatment plans based on patient progress and any emerging health concerns.
• Educate patients and caregivers regarding disease management, treatment options, and post-discharge care, significantly aiding their understanding and adherence to care plans.
• Coordinate with the inpatient team for safe hospital admissions and discharges, ensuring continuity of care across patient settings.
• Document all patient interactions meticulously within our electronic medical record system, ensuring that all care delivered is accurately recorded and accessible for ongoing management.
• Participate in quality improvement initiatives and clinical research protocols, contributing to the advancement of cardiac care practices.
• Demonstrate accountability by adhering to the scope of NP practice and ensuring the delivery of safe, competent patient care in all situations.
• Maintain a professional environment that promotes safety, therapeutic relationships, and exceptional patient experiences.

Patient Population / Client Focus

Your primary focus will be on patients with various cardiac conditions, including but not limited to heart failure, pulmonary arterial hypertension, and post-heart transplant care. You will be equipped to provide specialized management for patients being considered for advanced therapies such as outpatient intravenous medication support or mechanical circulatory devices. Understanding the complexities of these conditions will enable you to deliver nuanced and tailored care.

Qualifications

The ideal candidate will possess the following qualifications:

• Master’s degree in Nursing with a Nurse Practitioner focus, specifically trained in the care of adult patients.
• Current licensure as a Registered Professional Nurse and Nurse Practitioner in the state of New York.
• Board certification as a Nurse Practitioner.
• Current certifications in BCLS, ACLS, DEA, and NPI.
• Proficient in the use of electronic health record systems and familiar with Microsoft Office applications.

Preferred Experience

Applicants should ideally have:

• A minimum of two years of experience in a clinical setting, particularly within cardiology.
• Experience with managing patients requiring advanced cardiac therapies.
• A deep understanding of chronic and acute cardiovascular conditions and familiarity with patient care strategies in a hospital setting.

Compensation and Benefits

We offer a highly competitive salary range of USD 120,000 to USD 220,000 per year, commensurate with your experience and qualifications. In addition, our comprehensive benefits package includes:
