As a Physical Therapist in our home care setting, you will work directly in patients' residences, allowing for a personalized approach to rehabilitation. Conducting thorough evaluations of patients to assess their physical therapy needs and create individualized treatment plans.

Regularly tracking and documenting patient progress, adapting treatment plans as necessary to align with evolving therapeutic goals.

Training patients, their families, and caregiver teams on effective use of assistive devices and self-care strategies to promote independence.

Participating in ongoing professional education and contributing to collective knowledge through networking and participation in professional societies.

Your role will encompass a diverse patient population, including individuals recovering from surgery, injury, or chronic illnesses. A Bachelor's Degree in Physical Therapy.

A valid state license in Physical Therapy.

Proficiency in word processing and computer skills.
