You will work in a dynamic home care environment that promotes collaboration and individualized care. As part of your work setting, you will travel to patients' homes, providing you with a unique opportunity to see firsthand the difference skilled physical therapy makes in improving function and quality of life.

Evaluate patients referred by physicians to develop effective, personalized treatment plans.

Implement therapeutic interventions at patients' homes using techniques that align with their physician's plan of care, ensuring meaningful change in their conditions and functionality.

Provide treatment across a diverse range of specialties, including orthopedics, pediatrics, neurology, and sports medicine.

Educate and train patients and their caregivers to empower them with knowledge and strategies for effective rehabilitation.

Maintain a strong focus on customer service and contribute to the overall effectiveness of the care team.

Collaborate with other healthcare professionals to enhance patient outcomes through a multidisciplinary approach.

Your role will involve working with a diverse spectrum of patients, including individuals recovering from surgery, young athletes, elderly patients requiring mobility support, and children with developmental needs. The home care setting allows for personalized attention, enabling you to deliver therapeutic interventions tailored to each patient's unique context and environment.

Doctorate in Physical Therapy (DPT) from an accredited institution.

A current, valid state license in Physical Therapy.

A minimum of two years of relevant experience in physical therapy practice.

Proficiency in word processing and computer skills to document patient care and progress accurately.

A commitment to building rapport and relationships with each patient to assist them in exceeding their functional goals.

A genuine desire to connect positively with people and engage with the community.

Experience in home health care settings is highly valued.

Demonstrated ability to manage a caseload effectively while maintaining quality of care.
